Athletic Forms and Information
To participate in Farrington High School Athletics, the following MUST BE COMPLETED:
*Must be completed using a Chrome browser*
1. Complete Annual HCAMP Concussion Education
2. Turn in your own Athletic Participation Packets to the Athletic Training Facility (located in the stadium).
Only fully completed Athletic Participation Packets will be accepted. Partial paperwork will not be accepted.
3. Schedule ImPACT Baseline test with the Athletic Trainers
